All clinical notes, medication lists, physician orders, and care plans are safeguarded under federal HIPAA privacy and security standards.

Authorized Family Updates

We discuss your health and care progress only with you, your ordering physicians, and the family members or representatives you authorize.

Minimum Information Online

Our website forms ask only for basic contact details. Sensitive medical histories and policy IDs are handled directly by phone or clinical fax.

Access to Your Record

You have the right to inspect, request copies of, or request amendments to your home-health clinical record at any time.

1. Our Pledge Regarding Your Health Information

We understand that health information about you and your household is personal. Tucker Home Healthcare LLC is required by law to maintain the privacy of Protected Health Information (“PHI”), to provide patients with a written Notice of Privacy Practices upon admission to care, and to notify affected individuals following any breach of unsecured health information.

When care begins in your home, your admitting nurse or therapist provides the full written Notice of Privacy Practices and reviews your communication preferences with you.

2. How We Use & Share Health Information for Care

Under HIPAA, we use and disclose health information only as needed to support your care and household well-being:

  • Treatment & Physician Coordination: Our Registered Nurses, Licensed Practical Nurses, Physical Therapists, and Home Health Aides share clinical assessments, vital signs, wound-care progress, and rehabilitation updates with your ordering physician, specialist, or hospital discharge team.
  • Payment & Benefit Verification: With your consent, we confirm eligibility and submit required clinical documentation to Passport, Medicare, Medicaid, or private insurance plans so your visits are covered properly.
  • Family & Caregiver Communication: We keep the family members or representatives you designate informed about visit schedules, medication changes, and daily observations.
  • Healthcare Operations & Quality Assurance: Clinical supervisors review visit notes and conduct employee in-service training to ensure safe, consistent care across every home we serve.

3. Website Inquiries vs. Clinical Transmission

Our website contact form, referral form, and Care Guide are designed for a simple first introduction — asking only for your name, callback phone number, email address, and a general description of the help you are looking for.

Please do not enter Social Security numbers, full insurance policy numbers, or detailed medical records into a website form. When a physician’s office, hospital case manager, or family member needs to share discharge summaries, medication lists, or signed clinical orders, please use our direct phone line at (614) 796-0939 or our clinical fax line at (614) 515-2435.

4. Your Rights Under HIPAA

As a patient of Tucker Home Healthcare LLC, you have the right to:

  • Inspect and receive copies: Ask to view or obtain a copy of your clinical home-health record, plan of care, and billing records.
  • Request an amendment: Ask us to correct or add information to your record if you believe something is incomplete or inaccurate.
  • Request confidential communications: Specify how and where we contact you (for example, calling a specific phone number or speaking with a designated family member).
  • Request restrictions: Ask us to limit what health information we share with certain individuals, or restrict disclosures to a health plan when you have paid for a service out-of-pocket in full.
  • Accounting of disclosures: Request a list of certain non-routine disclosures of your health information made outside of treatment, payment, and healthcare operations.

5. Questions or Privacy Concerns
