2. Emergency Situations

This website and its online forms are not an emergency medical service. If you or a loved one is experiencing a medical emergency, chest pain, difficulty breathing, or immediate danger, call 911 or go to the nearest emergency room right away.

Current patients and families who need to reach an on-call nurse outside regular office hours (Monday – Thursday: 9:00 AM – 4:00 PM) should call our main telephone line at (614) 796-0939, where we maintain 24-hour nurse availability, 7 days a week.

3. How Home Care Services Are Initiated

Browsing this website, completing the Care Guide, or submitting a contact or referral form does not by itself create a provider–patient relationship or admit a patient to home care. Formal home healthcare services begin only after:

• Our Columbus intake team speaks with the patient, family, or referring clinician
• Required physician orders and medical authorizations are verified (when applicable)
• An in-home nursing, therapy, or personal-care assessment is completed and a Plan of Care is signed

We proudly serve households in Franklin, Delaware, Licking, Fairfield, Pickaway Counties and nearby Central Ohio communities. We reserve the right to decline or refer a request elsewhere if a service is outside our clinical scope, outside our staffing territory, or if a safe home visit environment cannot be established.

4. Insurance Coverage & Financial Responsibility

References on this website to Passport, Medicare, Medicaid, and Private Pay describe the payment programs Tucker Home Healthcare LLC accepts. Eligibility for specific skilled nursing, therapy, or personal-care visits depends on the rules of your health plan and medical necessity criteria.

Our intake coordinators verify benefits before care starts and explain any expected patient responsibility in plain language, as outlined on our Patient Rights and Insurance pages.

5. Acceptable Use & Intellectual Property

When using this website and its forms, you agree not to:
