Dignity & Respect at Home
Our nurses, therapists, and aides are guests in your home and treat you, your family, and your household property with courtesy and respect.
Voice in Your Care Plan
You have the right to help shape your goals, understand every scheduled service, and know the name and role of every clinician visiting your home.
Informed Consent & Choice
You may accept or refuse any treatment or visit after we explain what that choice means, and you may review or update advance directives at any time.
Safe, Confidential Grievance Path
You can raise a question or concern with our Columbus office or the Ohio Department of Health without fear of retaliation or reduced care.
1. Overview & Family Representation
Every person receiving care from Tucker Home Healthcare LLC holds the rights described on this page in accordance with Ohio Department of Health home-health regulations and federal Conditions of Participation. Before your first service begins, our clinician provides a written copy of your Patient Bill of Rights and reviews it with you in plain, understandable language.
A spouse, adult child, legal guardian, health care power of attorney, or any representative you choose may exercise these rights on your behalf whenever you ask them to or when Ohio law authorizes them to act for you.
2. Dignity, Privacy & Respect in Your Home
Home care takes place in your personal living space. Our nurses, therapists, and aides are trained to honor the routines, cultural traditions, and privacy of your household at every visit:
- Respectful treatment: You have the right to be cared for with warmth, patience, courtesy, and respect at all times.
- Protection of personal property: Your home, furnishings, and personal belongings must be treated with care and respect.
- Freedom from harm: You have the absolute right to be free from verbal, mental, sexual, or physical abuse, neglect, corporal punishment, or financial exploitation/misappropriation of property.
- Equal access to care: Care is provided without discrimination of any kind. Read our full Non-Discrimination Notice.
3. Active Participation in Your Care Plan
Your care plan is built around your goals and your doctor’s directives — never a one-size-fits-all clinic schedule:
- Planning your care: You and your designated family members have the right to participate in developing, reviewing, and updating your plan of care, including both hourly and live-in schedules.
- Advance notice of services: Before care begins — and whenever a change is proposed — we explain which disciplines will visit (Registered Nursing, Physical Therapy, Home Health Aide, or Personal Care), how frequently visits are scheduled, and what each visit aims to accomplish.
- Knowing your care team: You have the right to know the full name, credentials, and supervising clinician of every person entering your home.
- Qualified professionals: All services are delivered by appropriately licensed, certified, or trained personnel who participate in ongoing employee in-service education.
4. Informed Consent, Refusing Care & Advance Directives
- Right to accept or decline: You may accept or refuse any specific treatment, medication assistance, or scheduled visit after we clearly explain the medical consequences of that decision.
- Advance directives: You have the right to formulate advance directives — such as a Living Will or Durable Power of Attorney for Health Care — and to have our clinical staff honor those directives in accordance with Ohio law.
- Transfer and discharge notice: You will be informed in advance of any planned transfer, discharge, or completion of services, except when an immediate change is required for your medical safety or emergency hospitalization.
5. Confidentiality of Medical Records & Financial Clarity
Your clinical record is strictly confidential. We share health information only with you, the family members you authorize, your ordering physicians, and as required for insurance billing or health regulation. You may request to inspect or amend your clinical record at any time; see our HIPAA Notice for full details.
Before care begins, we verify your benefits under Passport, Medicare, Medicaid, or Private Pay and explain in plain language what your plan is expected to cover and whether any out-of-pocket cost applies.
6. How to Voice a Concern or File a Complaint
You or your family may voice a question, complaint, or recommendation regarding a visit, schedule, or clinician at any time. Raising a concern will never result in retaliation, discrimination, or an interruption of the care you are entitled to receive.

- Notify our office promptly if your health condition, prescriptions, insurance coverage, or schedule needs change.
- Maintain a safe home environment for visiting nurses, therapists, and aides, including informing us of pets or household hazards.
